Admissions & census

From the first referral call to a confirmed admission, with a census you can trust because it only counts residents who are actually admitted.

Available today in a demonstration environment with synthetic data.

Referral intake built for skilled nursing

Referrals arrive from hospital discharge planners and other sources. Intake captures what a SNF actually needs to decide.

  • Clinical packet collected and tracked with the referral.
  • PASRR tracking records Level I and Level II status. SNF OS records PASRR; it never decides it.
  • Clinical suitability flags for isolation, dialysis, bariatric, behavioral supervision, IV therapy, wound vac, oxygen, and trach or vent.
  • Payer captured as Medicare Part A, Medicare Advantage or managed care, Medicaid, Medicaid pending, or private pay.

The admissions pipeline

Each admission moves through six stages. At every stage it shows the stage, next action, responsible person, blocker and time in stage.

  1. Intake

    Referral and clinical packet received.

  2. Clinical review

    Nursing assesses suitability.

  3. Business office

    Financial review of payer.

  4. Decision

    A person accepts or declines.

  5. Bed & arrival

    Bed matched, arrival planned.

  6. Admitted

    Confirmed and counted.

Decisions are always made by people. SNF OS never admits or declines on its own. It shows the facts, records the decision and who made it, and keeps the audit history.

A census that means what it says

Census counts confirmed admissions only. Acceptances, bed holds and planned arrivals are shown, but they are never mixed into the number.

  • Occupancy by unit
  • Beds that are ready right now
  • Expected arrivals
  • Payer mix, visible only to financial roles
